After submitting our form, a payment window should appear.
RUSH evaluations must be paid online in advance. Those wishing other payment options should not use our video RUSH form but should choose another option.

If you have any questions, please use the contact us options below.

An email will be sent to use to set an appointment option to accept or ask for another option. (For fastest service, use our RUSH online/email option instead.)

Note: During the Video Evaluation call, the parent will share records of the child's learning during the live video call.
(Portfolio records are NOT shared in advance for video evaluations.)
The child must be present to listen in and/or join in the discussion of the live video call.

If you would rather share digital information in advance via emailed photos or scans or a link or such,
use our Online/Email option
instead.

  For a RUSH evaluation via live video call, use the form on this page.
With this option, NO records are shown prior to the call. Portfolio records are shown while on the call. The student(s) must be present during the call; it will be similar to having them sit in on a parent-teacher conference.

      $20 more each additional student

2. To begin an evaluation now & get feedback now; then finish evaluation 2-12 months later, as your child's evaluation deadline draws closer. Not available during the summer. Starts again September 1
